Illinois 4-H Shooting Sports Program
All 3, 4-H Forms need to be printed, completed, signed, submitted and fees up to date to participate in any 4-H organized shooting sports program practice or compete on the Hot shots 4-H Shooting Sports competitive team.
- 2022-23 4-H Club Member Enrollment Form – This needs to be returned to the office.
- 2022-23 4-H Volunteer Enrollment Form – This is for the active adult coaches. This needs to be returned to the office. No fee is associated with volunteers.
- Risk Form – Adults and members need to complete one of these forms in their 4-H career. When in doubt, please have them fill it out. This needs to be returned to the office.
- Medical Form – This form needs to be completed, housed in a binder, and kept on site of the classes in case of an emergency.
4-H's participation year ends September 30, Paper work should be submitted ASAP so the participants insurance coverage will have no gaps.
If a scheduled program happens to occur on both sides of the deadline, new forms may be required to continue eligibility for participation
